Services

Bereavement Care
Provided to the family for a period of 13 months, longer if needed. Care is offered by support groups, face to face visits, phone calls, and letters designed to meet the needs individualized for the family.
Certified Nursing Assistant
Assists with activities of daily living (bathing, grooming, skin care, manicures, light housekeeping chores, socialization) and provides caring comfort twice a week (more often if needed).
Social Worker
Visits with the patient and family to offer emotional support, facilitates verbal and tactile
communication, assists with finding resources for financial needs and funeral arrangements. Also helps explain the Medicare/ Medicaid system and benefits
Medical Director
Reviews the patient and family’s plan of care every two weeks and offers guidance and support for continued care while making visits when necessary. They do not take the place of the patient’s primary physician who continues to care for the patient.
Nursing Visits
Visits the patient twice a week (more often if needed) to assess the patient's physical needs (pain, fatigue, hydration & nutrition, difficulty in breathing, skin care, alertness, restlessness, depression, etc.). The nurse keeps in contact with the physician and family members to continually update the plan of care.
Chaplain
Assesses for spiritual needs and/or distress, provides opportunity for expression of beliefs, fears, and hopes, provides access to religious resources, assesses bereavement needs, and facilitates religious practices.
Special Services
Are also provided including sign, translators, wound care specialists, Alzheimer/dementia, and community education with our professionals providing services in the patients home, nursing center, assisted living
center, residential care facility, and/or hospital.
